Foundayo (orforglipron): the GLP-1 pill that isn't a peptide, and how a priority voucher sped up its approval

GLP-1 stands for glucagon-like peptide-1, so it's natural to assume every GLP-1 medicine is a peptide. Most are. Foundayo isn't. Lilly's once-daily tablet, approved by FDA on April 1, 2026, works on the same receptor as semaglutide and tirzepatide but is built from a small, man-made molecule instead of a chain of amino acids. It also set a speed record: FDA cleared it 50 days after filing, under a new voucher program the agency is still testing. Here's what the approval record says about the drug, the trials and the review.
Facts are as of October 31, 2026. Sources, with retrieval dates, are at the bottom.
The gist
- FDA approved Foundayo (orforglipron) on April 1, 2026 as a long-term weight-loss medicine, used with diet and exercise, for adults who have obesity or who are overweight and have at least one weight-related health problem. [V]
- Lilly and the trial authors describe orforglipron as a small-molecule, non-peptide GLP-1 receptor agonist. Semaglutide, the active ingredient in the other GLP-1 pills, is a peptide. [V]
- In the 72-week ATTAIN-1 trial of 3,127 adults without diabetes, the top orforglipron group lost 11.2% of body weight on average, against 2.1% on placebo. [V]
- It was the first new molecular entity approved under FDA's national priority voucher pilot (CNPV), 50 days after filing and 294 days ahead of its standard target date. FDA called it the fastest approval of a new molecular entity since 2002. [V]
- 50 daysfrom filing to approval, by FDA's count [V]
- 294 daysahead of the application's standard review deadline [V]
- 3,127adults randomized in the ATTAIN-1 trial [V]
- 902 vs 4,114molecular weight (g/mol) of orforglipron calcium vs. semaglutide, per their FDA labels [V]
Peptide or not? What the labels say
Semaglutide is a peptide by any definition. Its FDA label says it shares 94% of its amino acid sequence with the human GLP-1 hormone, and says "the peptide backbone is produced by yeast fermentation." [V] The label lists its molecular weight as 4,113.58 g/mol. [V] That's a big molecule for a pill, which helps explain why the first semaglutide products were injections. [S]
The semaglutide tablets solve the swallowing problem with an extra ingredient. Both the Wegovy tablet label and the Rybelsus label list salcaprozate sodium (SNAC) and describe it as an absorption enhancer. [V] The Wegovy tablet label also asks for an empty stomach in the morning. [V]
Orforglipron takes a different route. Foundayo's label gives the molecular weight of orforglipron calcium as 902.0 g/mol, under a quarter of semaglutide's. [V] Lilly's release calls it "a once-daily small molecule (non-peptide)" GLP-1 receptor agonist, and the New England Journal of Medicine paper on ATTAIN-1 uses the same "small-molecule, nonpeptide" description. [V] The acting head of FDA's drug center described it as a GLP-1 receptor partial agonist "which does not need to be taken on an empty stomach." [V] The label says once daily, with or without food. [V]
Lilly says Chugai Pharmaceutical discovered orforglipron and Lilly licensed it in 2018. [V]
Why does this matter on a peptide site? Because "GLP-1" now names a target, not a type of molecule. The approved GLP-1 medicines include peptides like semaglutide and tirzepatide and, since April, a non-peptide small molecule. When you see "GLP-1" in a headline, the ingredient behind it could be either. Our GLP-1 explainer sorts out the approved, compounded and research-labeled versions, and our peptide basics page covers where FDA draws the line between a peptide and a protein.
The trial record
Two 72-week, randomized, double-blind, placebo-controlled trials supported the approval, FDA said. [V]
ATTAIN-1 enrolled 3,127 adults with obesity and no diabetes in several countries, comparing three strengths of orforglipron with placebo on top of diet and activity. [V] The NEJM paper, published September 16, 2025, reported average weight changes at 72 weeks of -7.5%, -8.4% and -11.2% for the three orforglipron groups, against -2.1% on placebo. [V] In the top group, 54.6% lost at least 10% of their weight and 18.4% lost at least 20%, compared with 12.9% and 2.8% on placebo. [V] Waist size, systolic blood pressure, triglycerides and non-HDL cholesterol all improved more than with placebo. [V]
Lilly's own headline number is higher, 12.4%, because it estimates the effect if everyone had stayed on treatment; the 11.2% figure counts everyone randomized, whether they stayed on or not. [V] Both are legitimate ways to report a trial. They answer different questions.
ATTAIN-2 enrolled more than 1,600 adults who also had type 2 diabetes along with obesity or excess weight, according to Lilly. [V] We haven't read a peer-reviewed results paper for it, so we don't report its numbers here. [U]
On side effects, the NEJM authors said gastrointestinal effects were the most common and mostly mild to moderate, and that the overall pattern matched other GLP-1 receptor agonists. [V] Between 5.3% and 10.3% of people in the orforglipron groups stopped because of adverse events, against 2.7% on placebo. [V] Like other GLP-1 medicines, Foundayo carries a boxed warning about thyroid C-cell tumors on its label. [V]
“the adverse-event profile was consistent with that of other GLP-1 receptor agonists”
How the voucher sped things up
FDA launched its national priority voucher pilot, known as CNPV, in June 2025. [V] A voucher is tied to one company's application, can't be sold or transferred, and buys a faster review: FDA's program page puts the target at one to two months, against six months or more for its other priority programs. [V] The centerpiece is a one-day, "tumor board style" meeting where a multidisciplinary team reviews the application together. [V] Products qualify by fitting one of five national priorities, including unmet medical need, affordability and onshoring of manufacturing. [V]
Orforglipron got its voucher in FDA's second batch, announced November 6, 2025. Wegovy, Novo Nordisk's semaglutide, was on the same list. [V] Lilly's application reached FDA on January 20, 2026, according to the approval letter. [V] Approval came April 1. FDA counts that as 50 days after filing, the formal acceptance step; from the date FDA received the application, it's about 10 weeks by our count. [V][S] The application's standard deadline under the user-fee schedule would have been January 20, 2027. [V]
FDA said Foundayo was the fifth approval under the pilot, and the first for a new molecular entity, meaning an active ingredient FDA had never approved before. [V] By then the agency had awarded 18 vouchers and issued six decisions. [V]
The oral Wegovy tablet followed a slower road. Its approval letter shows Novo's application arrived February 28, 2025, and FDA approved it December 22, 2025, about 10 months later. [V][S] That letter doesn't mention a voucher. [V]

Questions for your clinician
General questions drawn from this page, to take to a clinician or pharmacist who knows your health history.
- Which products get vouchers, and who decides? FDA said its November 2025 picks came from outside applications and internal nominations by its review divisions. [V]
- What does a company agree to in return? FDA's commissioner has tied vouchers to affordability, domestic manufacturing or unmet need. [V]
- Could a peptide medicine qualify? FDA's program page says the pilot covers drug and biological product applications in general. [V] No peptide outside the GLP-1 class has received a voucher that we found. [U]

Why peptide readers should care about the speed
This approval shows FDA can move fast when it decides a product matters. Two months of focused review replaced a deadline almost a year out. That's an access win, and it came with a full safety label. [S]
Compare the pace on the compounding side. HHS's interim policy, published October 30, 2026, lets state-licensed pharmacies compound six peptides, including BPC-157, under set conditions. It lasts only until a final rule decides each peptide's place on the 503ASection of federal drug law covering compounding by state-licensed pharmacies and doctors, usually for a prescription written for one patient. Glossary entry bulks list. [V] The department that cleared a new obesity drug in 50 days still has no final answer on those six. [S] Our policy tracker logs each step, and our bulks list explainer shows how a substance gets on the list. Whether voucher-style speed could ever reach those reviews is an open question. FDA's June 4 public meeting on the pilot and the written comment period that ran to June 29, 2026 were the first formal chances to weigh in. [V]
The approval also says something about what "approved" requires. Lilly ran trials in thousands of people before filing, and that is the road no company has funded for most wellness peptides. Our piece on why most drugs in human trials never get approved covers the cost side.
Limits and unknowns
- FDA's "50 days" counts from filing, not from when it received the application. FDA's page doesn't give the filing date itself. [U]
- We cite ATTAIN-1 from its published abstract and Lilly's summary. We haven't reviewed FDA's full clinical review memo for Foundayo. [U]
- Head-to-head data between orforglipron and the peptide GLP-1s weren't part of the approval package we read, so we don't compare them on results. [U]
- The CNPV is a pilot. FDA hasn't said whether it will become permanent or how its criteria may change after the June comment period. [U]
The headline is simple: an obesity drug can now target the GLP-1 receptor without being a peptide, and FDA approved it in record time. What happens next with the voucher program is the part to watch.
Common questions
Is orforglipron (Foundayo) a peptide?
No. Lilly and the authors of the main trial describe orforglipron as a small-molecule, non-peptide GLP-1 receptor agonist. It acts on the same receptor as semaglutide, which is a peptide, but its molecular weight on the FDA label is about 902 g/mol, against roughly 4,114 for semaglutide. When did FDA approve Foundayo, and for what?
FDA approved Foundayo on April 1, 2026 for chronic weight management, alongside diet and exercise, in adults with obesity or with overweight plus at least one weight-related condition. Source: FDA press release, April 1, 2026
What is a national priority voucher?
It's FDA's Commissioner's National Priority Voucher pilot, announced in June 2025. A voucher, which can't be transferred, aims for a review in one to two months. FDA said Foundayo was approved 50 days after filing, the first new molecular entity cleared under the pilot. Source: FDA, CNPV pilot program; FDA press release, April 1, 2026
