Retatrutide explained: the TRIUMPH results, FDA status and why it can't be compounded

Ever heard someone call retatrutide "the next Zepbound" and wondered how much of that is hype? Quite a bit of it is backed by data. Retatrutide is the first drug to reach phase 3 that works on three gut-hormone receptors at once, and its trial numbers are the reason it shows up in nearly every weight-loss conversation online. It also isn't approved anywhere yet, and federal law doesn't let pharmacies compound it.
Facts are as of October 29, 2026. Sources, with retrieval dates, are at the bottom.
Updated October 29, 2026. On October 28, Lilly reported that retatrutide beat Ozempic on A1C and weight in TRANSCEND-T2D-2, a phase 3 trial that pitted the two drugs against each other in 1,298 people who have type 2 diabetes. Lilly's October 29 quarterly report highlighted the result and gave no new filing date. Full breakdown: retatrutide vs. Ozempic. [V]
The gist
- Retatrutide is Eli Lilly's investigational once-weekly GIP, GLP-1 and glucagon receptor agonist, often called a "triple agonist." [V]
- In the phase 3 TRIUMPH-1 trial, the highest-dose group lost an average of 28.3% of body weight at 80 weeks versus 2.2% on placebo (efficacy estimand), per Lilly. [V]
- It is not FDA-approved. Lilly's stated plan is a Biologics License Application, filed in the first quarter of 2027. [V]
- FDA says retatrutide "cannot be used in compounding under federal law," and the October 28, 2026 HHS peptide policy didn't change that. [V]
- 28.3%average weight loss, highest-dose group, TRIUMPH-1 at 80 weeks (RCT) [V]
- 2,339adults randomized in TRIUMPH-1 [V]
- 24.2%average weight loss, highest-dose group, phase 2 at 48 weeks (RCT) [V]
Three hormones, one molecule
Most of the weight-loss drugs people know act on one or two hormone receptors. Semaglutide targets GLP-1. Tirzepatide targets GLP-1 and GIP. Retatrutide adds a third: the glucagon receptor. [V] The peer-reviewed TRIUMPH-1 paper describes it as "a triple-receptor agonist" of GIP, GLP-1 and glucagon receptors. [V] The rationale the phase 2 trial authors described is that GLP-1 and GIP work on appetite and blood sugar while glucagon signaling may raise energy use and act on liver fat; that rationale is still being tested in people. [S]
It's a peptide, built from a chain of amino acids like the other drugs in its class. For the longer history, see our peptide medicines timeline from insulin to GLP-1.
The evidence, trial by trial
Phase 2: the first big signal (2023)
The phase 2 obesity trial, published in the New England Journal of Medicine in June 2023, randomized 338 adults with obesity or overweight. The highest-dose group lost an average of 24.2% of body weight at 48 weeks, against 2.1% on placebo. [V] Those numbers, from a randomized, placebo-controlled trial, are what put retatrutide on the map. A companion phase 2 trial in people with type 2 diabetes reported drops in both blood sugar and weight. [V]
TRIUMPH-1: obesity without diabetes (May 2026)
TRIUMPH-1 (NCT05929066) is the main phase 3 obesity trial: randomized, double-blind, 2,339 adults, 80 weeks. [V] Lilly's May 21, 2026 release reported average weight loss of 19.0%, 25.9% and 28.3% across the three dose groups by the efficacy estimand, against 2.2% on placebo. [V] Under the stricter treatment-regimen estimand, which counts everyone regardless of whether they stayed on the drug, the peer-reviewed paper reports 17.6%, 23.7% and 25.0%, versus 3.9% on placebo. [V]
Other findings stood out too. Among the highest-dose group, 45.3% lost at least 30% of their body weight, per Lilly. [V] In a blinded extension for people who began with a BMI of 35 or more, average loss reached 30.3% at 104 weeks. [V] The paper also reported less knee pain in participants with knee osteoarthritis and fewer breathing interruptions during sleep in those with obstructive sleep apnea, both compared with placebo. [V]
“In adults with obesity, retatrutide resulted in significant weight reduction.” [V]
TRIUMPH-2 and TRIUMPH-3 (July 2026)
On July 23, 2026, Lilly reported two more successful phase 3 trials. [V] In TRIUMPH-2, adults with obesity and type 2 diabetes in the highest-dose group lost an average of 20.8% of body weight, and A1C fell by an average of 1.5 points, with up to 40.0% of participants reaching an A1C below 5.7%. [V] TRIUMPH-3 enrolled people with severe obesity who already had heart or blood-vessel disease; they lost up to 22.6% on average at 80 weeks. [V] Lilly also reported lower triglycerides, non-HDL cholesterol, systolic blood pressure and hsCRP, a marker of inflammation, in TRIUMPH-3. [V]
People with diabetes usually lose less weight on these drugs, so analysts read those results as in line with expectations, BioPharma Dive reported. [S]
TRANSCEND-T2D-2: the first head-to-head (October 2026)
On October 28, 2026, Lilly reported that average A1C fell 2.2 points in both retatrutide groups and 1.8 on Ozempic at 80 weeks, and body weight fell 19.5% and 20.9% against 9.4%, among 1,298 participants with type 2 diabetes (open-label, efficacy estimand, topline only). [V] Lilly calls it the sixth positive phase 3 trial. [V] Our TRANSCEND-T2D-2 breakdown covers the side effects and the caveats. TRIUMPH-5, against tirzepatide, is still running. [V] Lilly's trial program also covers chronic low back pain, kidney and heart outcomes, and fatty liver disease (MASLD). [V]
Where retatrutide stands with FDA
FDA hasn't approved it. Lilly's answer to "Is retatrutide approved by the FDA?" is a flat "No," and the company calls it an investigational medication. [V] Lilly says it's completing the chemistry, manufacturing and controls package and intends to send FDA a Biologics License Application in the first quarter of 2027. [V] Lilly had earlier listed retatrutide among possible 2026 submissions, then told CNBC it needed more time for manufacturing and quality data, according to BioPharma Dive. [S]
The BLA detail catches people's eye, because many peptide drugs go to FDA as new drug applications. Lilly's own releases name a BLA. [V] We haven't seen Lilly or FDA explain the choice of pathway in a public document. [U] No FDA decision date exists yet, since nothing has been filed. [V]
Can retatrutide be compounded?
No, not under federal law today. A 503ASection of federal drug law covering compounding by state-licensed pharmacies and doctors, usually for a prescription written for one patient. Glossary entry pharmacy can compound from a bulk ingredient only if it has a USP or NF monograph, is part of an FDA-approved drug, or is on FDA's 503A bulks list. A 503BSection of federal drug law for outsourcing facilities: larger compounders that register with FDA and must follow drug-manufacturing quality rules. Glossary entry outsourcing facility needs the ingredient on the 503B list or a drug on FDA's shortage list. [V] FDA told the National Association of Boards of Pharmacy in a March 31, 2025 letter that retatrutide meets none of these. [V] FDA's current GLP-1 page repeats it: "Retatrutide and cagrilintide cannot be used in compounding under federal law." [V]
The HHS enforcement discretion policy released October 28, 2026 covers six other peptides, not retatrutide. [V] Our news analysis of that policy and retatrutide goes through it line by line. FDA has also sent warning letters to sellers of retatrutide, calling the products unapproved new drugs. [V] If the legal categories are new to you, our compounding categories explainer and GLP-1 status guide lay them out.
What people get wrong
"It's basically approved, just waiting on paperwork." No application has been submitted. Lilly's stated plan is a filing in the first quarter of 2027, and approval is never automatic. [V]
"Research-labeled retatrutide is the same molecule." Lilly says retatrutide is legally available only in its trials, and that illicit products may hold the wrong ingredient or the wrong amount of it. [V]
"The new HHS policy covers all peptides." It names six. Retatrutide isn't among them. [V]
Limits and unknowns
- Side effects were mostly gastrointestinal. In TRIUMPH-2, Lilly reported diarrhea, nausea, constipation, decreased appetite and vomiting more often than on placebo, plus more dysesthesia, an unusual skin sensation. [V] Lilly described them as generally mild to moderate. [V]
- More people stopped treatment for adverse events at the higher doses in TRIUMPH-2 than on placebo, though not at the lowest dose. [V]
- In TRIUMPH-3, the heart-event analyses were inconclusive. MACE-5 events numbered 44 on retatrutide against 52 on placebo, and MACE-3 events 27 against 23, both with confidence intervals crossing 1. [V] BioPharma Dive noted the open questions about extra heart benefit from a triple agonist. [S] Dedicated outcome trials are ongoing. [V]
- All the phase 3 trials are Lilly-funded. Full peer-reviewed papers for TRIUMPH-2 and TRIUMPH-3 weren't published when we checked. [V]
- Long-term data beyond about two years, and data on what happens after stopping, aren't public yet. [U]
